How are you modelling the answers given to the questions? One possible
approach might be to use a field like "answer" to record the selected
multiple choice answer, and to use the "text" field of the tiddler to store
the additional information? One would link the answer to the corresponding
question with a "question" field that contains the title of the question
tiddler to which this one is an answer.
