Using your example I would create these tiddlers

Tiddler Title: "Medical"
    tag: "category"

Tiddler Title: "Name of Condition 1"
    tag: "sub-category"

Tiddler Title: "Name of Condition 2"
    tag: "sub-category"

Tiddler Title: "Treatment"
    tag: "sub-sub-category"

Tiddler Title: "Specialists"
    tag: "sub-sub-category"

Tiddler Title: "Documentation"
    tag: "sub-sub-category"

Give each of the tiddlers a nice color refecting the category level. So for 
example: "Medical" in dark blue, Condition 1 & 2 in a medium blue and the 
rest in light blue.

Create one temporary tiddler with any name and ALL Categories, 
sub-categories and sub-sub-categories as tags. This is just so that you 
prepare the tags and they will appear in the list of tags.

Now create, as an example a tiddler which should be a documentation.

Give it any title and apply the tags "Medical", "Name of Condition 2", 
"Documentation".

That's it.
