Like Jeremy says, the documentation you've read is most likely for the TWC (found at classic.tiddlywiki.com). Attention is not primarily on TW5 at tiddlywiki.com. The purpose of the MainMenu Is/was a sidebar on the left side that you could easily modify while leaving the right sidebar primarily for "system features". TW5 has been designed more with the "new platforms" in mind, ipad, smartphones etc and has no MainMenu, only a sidebar. However, you can rather easily modify the sidebar, for instance add a tab and put a menu under this. Maybe using the tableofcontents macro.
