hmmm... changing "PageTemplate"... and doubled title/subtitle.... sounds 
like TiddlyWiki *Classic*... not TW5.

Please confirm which version of TiddlyWiki you are using.

Regardless, one possible solution is to add a floating image to the 
SiteTitle content.

In TW5, you would write:
   @@float:left;[img[http://placekitten.com/40/60]]@@ site title here

In TWClassic, you can use:
   [<img[http://placekitten.com/40/60]] site title here
