I remember experiencing the same difficulties as you've had, when I first 
started using the *<$appear>* plugin. I created my own copy of the 
documentation for it here

and you're welcome to make use of it. I have a used a macro and stylesheet 
to generate those fake code boxes, and it requires that I escape the 
quotation marks / speechmarks. If you intend to make use of my examples, 
make sure to replace the curly quotes with straight quotes first. A simple 
find / replace would do it.
