Mark, Are you using windows? There are some problems saving the settings. When you update the settings, and click the button to save them, it is supposed to update the json file. It may not be on non-linux systems and I haven't been able to figure out why. There is also a bug where tiddlers don't always save the created and modified fields correctly that I don't understand.
You can add an existing wiki by giving it a name and giving the correct path in the settings.json file.
