I'm unsure why you have two separate macro calls in your example. 
Regardless, maybe this works:


\define image-inner() [img width=80 [$(caption)$|$(tiddler)$]]

\define image(tiddler)
<$vars caption={{$tiddler$!!caption}} tiddler='$tiddler$' >
<<image-inner>>
</$vars>
\end
