If you are using the node version I made a plugin to sort of do this. It 
can intercept the import process and save the image or whatever media is 
imported to a folder and create a _canonical_uri tiddler for it. I don't 
think I have written much documentation for it though. 

The plugin is here, if you are feeling brave. Make sure to back up all your 
data.

I am pretty certain that this wouldn't be possible to automate in any 
reasonable way in a single file wiki.
