Hmm. Just thinking how to do this test without doing a full re-install. If I were to disable your plugin and then change the auth method to Basic - would that test your scenario.
I'm very confident about the 'one shot' save when using Windows Auth - it was a clear pattern - restart server - first save goes well, any subsequent doesn't.
