Interesting thought. So really what you are talking about is a vertical of TiddlyWiki where you create diagrams, as you create shapes - whether they look like circles, rectangles or triangles or whatever you can click on the shape and it will open a tiddler where you can write annotations. As you draw lines between objects they become tags.
It's an interesting idea (even if that's not what you are getting at :)) I don't think it would be too difficult either. When I package up GeoTiddlyWiki in a way I'm happy I'll make sure I take a look!
