Correct. Most people leave the core as is and then supplement their TW
with plugins and extensions[1]. If they feel that a core function
needs editing or amending then they will usually override[2] it. If
they feel their overriding method is better and suitable for the core
they can create a ticket for it to be included into the core (that
process will be made easier once the move to github happens[3]).

There are some tools that are used to assemble the core code and make
the TW file. These are called Cook and Ginsu [4]
