Theoretically, what you really want is a version of Xvnc that understands Xinerama and can be configured such that it exports multiple screens, each with a different size, and allows each screen to listen on a different VNC server port. How to do that is best left as an exercise to the reader, though. The disruption to the code would be very deep, and thus I doubt anyone in the community would be interested unless there was a more ubiquitous need (and preferably a commercially-identifiable one) for such a solution.
