I'm currently looking into the waterway rendering in osmarender for unrelated reasons (support for waterways in tunnels and minimum width) and the rules are really strange.
What I have so far is in http://trac.openstreetmap.org/attachment/ticket/1294/waterway-tunnel-v2.patch Width is not fixed yet.
