It actually has a gigabit Ethernet, not connected over USB.

The RPi-4 doesn't "need" a heatsink. It's design is intended to provide high-performance bursts to improve the perceived speed - i.e. to create a better user experience. It has throttling if the CPU temperature gets too high, but it you need to run it continuously at full speed then you may need a fan. A number of fans are available, for example:
