Server 67.64.199.49 seems to be off by more than 200 ms currently,

It's not today. :-)
For a dumb sntp client 200ms on an average desktop computer is plenty
accurate. An ntpd configured with 3 or 4 servers will know to just
ignore it.
That being said, this is one of the few edge cases that should be
pushed out with stricter limits in the scoring system.
We really need more servers in the Global pool before I do that though:
