Thanks Fabien.  So now I am a little further.  I have the inherited form 
showing everywhere but there seems to be a problem with position="replace".  
What I am seeing is that when I replace a page in the invoice notebook, I 
actually get 2 pages of the kind being replaced.  I cannot see why this should 
happen.  The second page is correct but the first page is missing buttons.  
Very strange.  Still investigating...
