There is no version check in the modules management which is quite rough.
Your procedure seems correct, but it is the first time I read it.
Did you use --update=all before connecting ?

not exactly.
if the config file doesn't exist, it is set to "admin"
when the config file is found, it is read from the config file

this has been discussed several times. It is a fast path to explore openerp. It 
is not upgrade-safe up to now.
To manage upgrade process in an easier way, I work with sources and xoe script 
I wrote for my hosting needs (debian-lenny recommanded). You may also consider 
moa from camptocamp. Or do it by hand.
Nevertheless, this doesn't explain why you failed between 0.11 and 0.12.
