Hi,
I guess you are using USB active cables and hubs in your testbed. It is highly likely that the reason is quality of some of the USB cables is not good. Such problems were solved in Indriya testbed by using ATEN's USB active cable. As Wasif suggested, you can first change the OS and if the problem still persists then you can choose to replace USB cables. In addition, please make sure the hub you are using is also of good-quality.
