If you have any suggestions as to procedures/tricks you think might trigger this bug I can certainly attempt to do so in the lab. Obviously we can't attempt to reproduce it on the customer's (live) system.

We have not seen this occur in our lab. For what it's worth, it occurred while the process was in TASK_UNINTERRUPTIBLE.
