I'm sorry, in my previous post regarding the "MUST BE TWO" contacts and the fed. regs;

(7) An explanation of whom to contact for answers to pertinent questions about the research and research subjects' rights, and whom to contact in the event of a research-related injury to the subject

I should have indicated that my understanding is that the contact for both questions and injury can be (and most frequently is) the same person.
