I actually have students give each team member a participation score of
0-100%. A student's grade is the score on the project multiplied by his/her
average participation score. I have them comment briefly on the rationale
for their score. Most of the time scores are very consistent. If someone is
way out of line, in the rating, I adjust. This doesn't happen very often,
though.

Hi Patrick-

In my research methods course students assign themselves to groups of 2
to 4 people. To encourage equal participation among group members I
advise everyone at the begining of the semester that they will be asked, 
at the final exam, to list every member's percentage of effort toward
their common project.  Students are told that this information is NOT
confidential and may be used to adjust the grades of students who
attempted to "hitchhike".  Students are reminded of this policy
throughout the semester and they are encouraged to ask one another if
they are pulling their fair share of the load.  Most of the time the
groups report equal contributions, but when it appears that one group
member has significantly under-contributed I reduce his or her
grade.  So far this has worked well and I get few complaints about
underperforming group members.  However, I think that the most important 
part of this strategy is prophylaxis i.e. encouraging students to talk
openly with one another about how fully they have contributed.
