The Canadian team of Pierce and Epling have done a fair bit of work on this, and they discuss the topic in the second edition of their text _Behavior Analysis and Learning_ (1999). Their basis paper is Pierce and Epling (1991), but the book it's in is probably not on everyone's shelf.
