The Canadian team of Pierce and Epling have done a fair bit of work 
on this, and they discuss the topic in the second edition of their 
text _Behavior Analysis and Learning_ (1999). Their basis paper is 
Pierce and Epling (1991), but the book it's in is probably not on 
everyone's shelf.  They also have a book out which may have some 
information on possible therapy (haven't looked at it myself). It's 
"Solving the Anorexia Puzzle (1991).
