Obviously, your institution's and departmental policies need to be followed. Aside from that, for all my exams I let students who, for whatever reason, are not there on the day of the exam take the make up in our testing center. Student's taking the in class test are given a "throw away" 5 point bonus question (usually syllabus items). The test is available for one week after the scheduled test. On rare cases (prolonged illness or injury to the student or family member) I extend the test for a day or two.
