The disorders in the DSM are not categorized based on their etiology, but rather on the observable or reportable symptomotology. Thus, mental retardation--the disorder--is defined, but the causes may be many (including alcohol ingestion during pregnancy). That's why you won't find Downs Syndrome, Fragile X, etc. in the DSM either.
