First you need one hundred blood samples, half of them from people known to have cancer. Then you reinforce your dog for emitting a specific behavior (sitting in this case) when it sniffs a cancer sample, and not when it sniffs a control sample. The training must be followed by testing in which which the tester does not know which samples are from cancer patients in order to control for the Clever Hans effect.
