The one thing I did find surfing around the web on my own is this website:

They have sections for all disciplines, including psychology! I only found one I wanted to use immediately but from what you describe there may be more that suit
you.

Also, there is one link from the division 2 (OTRP) website by Patricia
Keith-Speigel that wasn't exactly a case report, but provides you with
scenarios she and co-authors used for research on student's perceptions of
ethics violations and I thought there were a few elements there that I could
adapt/adopt for use.
