The official DSM-IV form is Attention-Deficit/Hyperactivity  Disorder. My 
sense after years of reading the literature indicates that  this form, however, 
is probably the one used less frequently in  peer-reviewed publications; more 
common usages are with hyphen, but no slash and  no hyphen or slash. The 
subtypes studied are, however, provided explicitly in  the papers' content. (No 
data 
on this.)   =)
