I suspect that all three of these conditions are more alike than
different.  One speculative characterization would be that they
all involve some kind of inhibitory dysfunction.  That manifests
itself as: (1) a failure to inhibit distracting events and
thoughts in ADHD, (2) a failure to inhibit a consuming or
habitual thought or action in OCD, and (3) a failure to inhibit
affective and behavioral effects of arousal in mania.  A number
of years ago, Gorenstein and someone else wrote a nice Psych
Bulletin (or Review, I forget) article arguing for a general
inhibitory dysfunction in many clinical phenomena.  I think it is
still a sound idea.
