I know of some grad programs that use exams at the end of two years to decide 
whether or not you get nothing and have to leave (fail), a master's and have to 
leave (a "Master's Pass"), or can go on to finish the PhD (a "PhD Pass").

I don't know how common it is, though.  Where I went to school there was no 
terminal Master's degree, but they would give you a Master's after you defended 
your dissertation proposal and completed your second-year research project and 
presented it.  If you were to leave after that (either willingly or because 
your advisor kicked you out), you'd take that Master's with you.

Have you been in grad school when the department has decided
not to allow a grad student to continue in the doctoral program but
decides to grant that student a  so called "terminal" Masters degree?
I have known a couple  of people that this has happened to and they were 
pissed. I guess they can pursue the doctorate at another institution.
I am curious as to the various reasons that may  lead to such  academic fatal 
decisions.
