Although it is not spelled out, according to examples in the Guide it seems that you do not include the country when the book is published in the USA, for example, New York, NY: Wiley. However, according to one example it should be London, England: Oxford University Press. In other words: Athens, Greece: Papazisis Publishers ; Cape Town, South Africa: Kruik Publishers; etc. However it is : Upper Saddle River, NJ: Prentice-Hall . Just for interest sake: I wonder how many foreign students know where Upper Saddle River is and what NJ stands for (most foreign/non-English speaking students will probably say "New Jork") ☺ .
