If you have the /var/log/qmail directories, it suggests that you or someone else on the machine began the qmail installation following the Life With Qmail instructions, but ended up just using syslog instead of the recommended multilog. It's not a big deal. >From the log you gave, the confirmation reply seems to get to TMDA just fine, but it doesn't recognize it, so it tucks it into the pending directory again and sends another confirmation message. Could you post your virtualdomains file? Also, is qmail installed in /var/qmail or somewhere else?
