I had nearly the same problem yesterday. Had to uninstall TMDA from a user, clean everything up, re-install, and re-configure the details. Then it was fine.
I have no explanantions. And the black text at the very bottom (that usually provides the hint of what stupid thing I'd done) was different from yours. Mine was making key complaints.
