Well, it sounds like that's the problem. What about those permissions? The lines below:

Notice the missing READ permissions. Perhaps the software could not read that .qmail-default... I realize that is a different user than the one you said you tested, but... Obviously, if the regular email works, but the 'extended' email does not, then something about the .qmail-default for that account is not working. Either the file cannot be read/used, or it isn't configured right... I don't understand how you have your email setup. It looks like there are .qmail-xyz FOLDERS from what you've shown me. I've never done it that way. I do know that Qmail looks for all the .qmail files in the root of the users home directory... Check to ensure that Qmail is configured properly before trying to fix TMDA. Qmail should be able to find the .qmail files for each user...
