Modifying Templates

I've been trying to figure out how to change the header that ht2html 
prepends to all .ht files.  Any pointers on where to look?  I'd like to 
make it a little more "custom" looking..

Also, is it possible to have ht2html include the results of an 
appliaction (or a file)?  Ideally, I'd like to be able to integrate it 
with my dynamically generated menus.
