The lists are traversed in the order specified in ~/.tmda/filters/incoming (the location of this file is in turn specified in ~/.tmda/config). So if you like you can put the blacklist first. Although this may have some unintended consequences if other addresses are matched both by the blacklist and the whitelist; you may drop mail that used to get through. Other ideas to consider: 1) use a more sophisticated pattern to match lists (see http://www.tmda.net/filter-sources.html#email_address) 2) use a keyword address for your medical mailing lists 3) prefilter before tmda for mailing lists (this is what I do) using e.g. procmail, exim-filtering (there are many others, those two I know how to use).
