I might have a suggestion...
Try removing PENDING_DELETE_APPEND from your config.
If you release a message from tmda-cgi, I'm wondering if the command to
release the message is then doing a delete on that message, and then
because the variable is set, that address is then being added to the
blacklist because of the way you have that variable configured.
Jason or Gre7g might will have a better clue than I do about exactly
when that variable is used, but I would give it a try as a test and see
if that helps.
