make sure you have confirm and debug logs turned on in the tmdarc file. 
This has helped me w/ the few problems I have come across.

I would also look at the logs for procmail and postfix to make sure they
are still doing there job.  can't say to much about procmail or postfix.
I stopped using them about 2 years ago when we made the switch to qmail.
