Yeah, I use greylisting as well -- I've replaced qmail-smtpd with qpsmtpd. It has a number of anti-spam plugins, including a greylisting plugin. I'm a very low-volume server, so I don't know how it performs under heavy load, but it's really nice. All plugins have to approve a message before the mail is even accepted at the SMTP level. That includes a SA scan and an Amavis scan. The delivery plugins can pass to other backends than qmail, but that's all I've used it with.

I have to admit, I've had a few "false positives", though. There are some servers that don't retry -- or that don't retry for a day or so. The former are mostly automated, e-mailed confirmations of things, or resending passwords. Provoke a second e-mail on the website and you're fine, provided you know what the hang-up is. It's kinda amusing -- I've got one spammer that delivers every 24 hours. Always, the previous greylist entry has timed out and the mail is again deferred.

It still doesn't catch everything. TMDA is that last line of defense. However, it catches enough that I have a listing of my TMDA queue mailed to me every few days, because very little spam makes it that far -- that's a level that I can watch without being glutted.
