I've checked in my home: I've only

.qmail-default -> .qmail
.qmail

and cat .qmail:

| /opt/local/tmda-1.0.3/bin/tmda-filter ./Maildir

I've tried different config and several test: in the end the following
.qmail:

| preline /opt/local/tmda-1.0.3/bin/tmda-filter ./Maildir

seems to solve the problem!

I've a couple of question:
1) your qmail assign file use "+alias" or "=alias" ?
2) have you a "preline" in .qmail ?
