Chad,

Files only get deleted from the pending queue on a sporadic basis. Well
actually it's based on a statistical average which you can set via
parameters within your .tmda/config file by changing the value of the
PENDING_CLEANUP_ODDS parameter which defaults to .01 (1%) meaning that files
will be removed on average once for every 100 emails that you receive. See
http://www.tmda.net/config-vars.html#PENDING_CLEANUP_ODDS for more info on
this parameter. This is the same parameter that controls removal of old
messages from the pending queue when they have reached an age equivalent to
or greater than the PENDING_LIFETIME parameter.

Last I knew when you are using tmda-pending to sort the messages in the
queue, any action you take other than show and pass simply modifies the name
of the file in your pending directory by appending :3,x where x is an R for
release and may have other values for other actions (not necessarily the
same letter used to perform the action). In this way tmda-pending knows the
disposition of any of the messages in the queue even if they have not yet
been removed from the queue.
