Is your qmail has the qmail-queue patch?

I'm using qmail and qmail-scanner with TMDA.  Below is my setup

My qmail is installed in /var/qmail.

in /var/qmail/bin

the original qmail-queue from qmail is renamed to qmail-queue.orig
I have a link qmail-queue to qmail-scanner.pl. (This ensures all mails go
through qmail-scanner).
qmail-scanner.pl passes mails to qmail-queue.orig (configured inside
qmail-scanner.pl).

Most likely you are using qmail-queue patch to define the location of
QMAIL-QUEUE in the  qmail smtp env which is set to qmail-scanner.pl.
That's why it is working when you send mail through the normal way where
as,  tmda-ofmipd has no knowledge of this environment variable and by
default passing mails to /var/qmail/bin/qmail-queue
