I don't really know the answer, but it sounds like there's some kind of
crash going on that prevents TMDA from performing the actions it needs to.

I would check these locations for possible error messages for clues:

* TMDA's log files (~/.tmda/logs/* - especially logs/debug)

* Web-server error log for any CGI trace-backs etc.
