Does your in.log contain log entries that show "Actn: CONFIRM pending"
or similar? It is possible that you are not generating challenges for
some reason. Even if you are, you may not be sending them properly;
check your MTA log as well for matching entries.
In particular, make sure you have a line that sets BOUNCE_ENV_SENDER to
something other than the null envelope address in .tmda/config, as a
null envelope is often blocked by ISPs and will prevent you from seeing
any bounce messages from TMDA.
