Yup. I'm pretty comfortable with the implementation I just checked in.
It checks out conceptually, and stood up to a 10,000 test-message
burst just fine.

Note, I discussed this with Charles Cazabon first since the original
maildir delivery code was taken from getmail. We really didn't agree
on the issue, so at this point our maildir delivery code is divergent.
He went with microseconds+pid as he feels the problem is more
theoretical, and something that should be solved by the MUA.
