I don't know how your system is set up, but ask yourself if the user has sufficient privileges to update the whitelist file. If e-mail comes in for an alias with qmail, I believe, TMDA will be launched as user "alias". Unless your whitelist is modifyable by everyone, user alias, or group nofiles; then the update will fail.
