When I first setup TMDA I had a similar problem.  I too prepared a patch, but 
it turned out that by running the mail through SpamAssassin, it was stripping 
out all 'Return-Path' information.  Adding the following to my global 
procmailrc file fixed it without needing a patch.  TMDA really needs the 
'Return-Path' to come through or bad things will happen.
