It's what I use. The plugin structure gives it the flexibility to handle a number of different situations. I can't speak to scalability personally, but I know there's been a lot of discussion about that on the mailing list. I let the qpsmtpd plugins weed out everything that's obvious and decline at the SMTP level, then qmail delivers to TMDA to hold, challenge, or deliver the rest.
