This is the default, but my users get pretty upset when legitimate mail is incorrectly labeled as SPAM with lots of asterisks.
I had an important client go absolutely bonkers over it. I still don't
know what about it triggers a normally sane person to become unhinged by
"***SPAM***". It seemed like a good way to get people familiar with
what was going to happen if they filtered their mail on the spam
headers, but wasn't worth it.
"report_safe 0" puts the info in the headers, where I think it belongs. That way, you can set your mail client, or maildrop (for site wide perhaps), to look for the X-Spam-Flag/Status header instead of the subject line. Seems to be a middle ground to keep everyone happy.
