Do you have tarpitting enabled?
Look for files tarpitcount and tarpitdelay in /var/qmail/control.
IIRC, after <tarpitcount> recipients, qmail-smtpd will delay
<tarpitdelay> seconds between each address, and might even increase the
delay for every recipient.
