Recompile the tcp.smtp file with only:

:allow,QMAILQUEUE="/var/qmail/bin/simscan"

Remove all other access controls.

The above will only allow your server to recieve/relay emails for
local/virtual domains. Nothing else will be relayed unless authenticated.
